How these pages were selected
The first indexable set uses two gates: meaningful CMS data volume and a buyer-facing IDR angle. That keeps the site away from thin programmatic pages and gives each page enough data to answer a real reimbursement question.
- States, payers, and specialties are limited to high-volume federal IDR markets.
- Service-code pages need at least 5,000 CMS disputes to enter the sitemap.
- Lower-sample pages can exist for users, but they stay out of the sitemap and carry noindex until the record is deep enough.
